Fill out the BIRTHDAY CLUB FORM HERE for each child! Kids -Ages Newborn to 17 yrs old. Only for the Lake County area and surrounding suburbs.
Each child will get a birthday card and something special on their birthday. Birthday Club Starts January 1, 2020. Signup today to be on the list for next year.
